Need to make Corrections

Once you submit your form, you cannot make any corrections to your form. You will need to go to the passport office on your day of appointment with a supporting document for the necessary changes to be made.

Unavailability of Witness

If your witness is unavailable, kindly print the appointment sheet and ask a commissioner of oaths at any Post Office to do the corrections.
At the PAC, alert the officers who will make the change in the system before the approval process.

Frequently Asked Questions and answers

Inability to Rebook Appointment (from Ghana.gov)

To be able to rebook, you have to wait for the date you chose for your first appointment to end before you rebook.
If you have already been to the passport office but could not fully process your application due to some documents you did not have on you, you cannot rebook. You can go to the passport office and complete the application. (Kindly use Google Chrome if you have any issues with rebooking)
Kindly follow the following steps to rebook if you have not already been to the passport office:
Visit https://www.ghana.gov.gh/
Click on Passport Office
Click on Apply
Click on Sign In to log in to your account
After successfully logging in to the account, you will be redirected to the passport site to enable you to rebook the appointment.
Click on My Application
Click on “History”
Select “Rebook Application” and select the application you want to rebook.
Select a “Passport Application Center” (PAC) of your choice
Select “New appointment date” [Choose available date shaded white]
Click on “Book Appointment”.
Click on “Print Slip” to print the new appointment date.

Materials needed before you apply for Ghana passport

FIRST TIME APPLICANTS

What you need to apply for the passport:

1. Birth Certificate(Proof of Ghanaian citizenship)

2. Documents to support Identity such as Driver’s Licence, National ID card

3. Proof of Profession (An introductory letter from the applicant’s employer that is if the applicant is currently employed or a work ID card that bears applicant’s name, organisation’s name and position. A student ID if the applicant is currently in school. Homemakers/housewives and applicants that are unemployed and those in the non-formal sector of employment like mechanics, hairdressers seamstresses, traders etc. do not need proof. Registered Business Owners and self-employed applicants must produce their business registration certificate as proof)

The Ministry of Foreign Affairs and Regional Integration (MFARI) has good news for you.

A Portable Document Format (PDF) version of the paper-based (Manual) Passport Application Form has been introduced to applicants on the Ministry’s Portal

The Introduction of the PDF Form is part of measures to make Passport Forms readily accessible to all Ghanaians. This completely eliminates middlemen popularly known as ‘Goro boys’ whose illegal activities have perpetuated bribery and corrupt activities in the passport acquisition processes. The PDF form will therefore replace the old paper-based application form.



Here are steps to follow in accessing the PDF Passport Application Form;

i. Visit the Ministry of Foreign Affairs and Regional Integration website – www.mfa.gov.gh or www.passports.mfa.gov.gh

ii. Click on Passport menu

iii. Read instructions and click on download passport forms

iv. Click on make payment and follow steps

v. A voucher PIN would be instantly sent to the applicant by a text message

vi. PIN should be written on the space provided on the printed form

vii. Submit completed application form with relevant documents to preferred Passport Application Centre for processing

The public is hereby to note that payment of the processing fee for the PDF form can only be made electronically and not through the banks as was done previously. Payment can only be made by mobile money (MTN, Vodafone) and Visa or MasterCard.
