5. Details of the document being requested, such as:
a.) Birth Certificate – Full name of document owner, date and place of birth, and name of parents;
b.) Marriage Certificate – Full name of husband and wife, and date and place of marriage;
c.) CENOMAR / AOM – same information as for birth certificate.

c.) Original copy of the duly signed Authorization letter or SPA that matches the signature in the accompanying valid ID or Identity Document of the document owner. (If the SPA has multi-purposes, include a photocopy of it.)

BLCI would like to inform you that the temporary sources of our electricity are from Power Barge 104 at Tapal, Ubay, and at Bohol Diesel Power Plant (BDPP) at Dampas Dist., Tagbilaran City. Considering that both Power Plants generate using DIESEL, which is costly, it will have an impact on the Generation Charge which is a pass-through charge to consumers of an electric distribution utility.

To those energized consumers, you are hereby encouraged to use/consume electricity wisely to avoid an electric bill shock in the next billing period.

BREAKING: The Tagbilaran-Garcia Hernandez 69-kilovolt line of the National Grid Corporation has been energized at 6:02 p.m., said NGCP Visayas spokesperson Betty Martinez.

All three distribution utilities, BLCI, BOHECO I and BOHECO II have started to restore partial power services to limited areas under their franchises.

For BLCI, it will only provide electricity to essential establishments in Tagbilaran City. The distribution utility is also still conducting tests to determine their facilities' stability leading to possible power loss.

The transmission of power from a barge in Ubay to Tagbilaran City was made possible after the Alturas Group of Companies through its Marcela Farms contributed power load of 1.2 megawatts to help meet the demand of 4 megawatts to stabilize NGCP's transmission lines. | More details to follow
