1. 𝑽𝒂𝒍𝒊𝒅 𝑷𝒂𝒔𝒔𝒑𝒐𝒓𝒕 𝒘𝒊𝒕𝒉 𝒉𝒐𝒍𝒅𝒆𝒓’𝒔 𝒔𝒊𝒈𝒏𝒂𝒕𝒖𝒓𝒆 🪪
- Must have at least 6 months of validity from the intended date of travel. (Hindi pwede yung malapit na ma-expire ha!)
- Should have at least two blank pages.

2. 𝑽𝒊𝒔𝒂 𝑨𝒑𝒑𝒍𝒊𝒄𝒂𝒕𝒊𝒐𝒏 𝑭𝒐𝒓𝒎 📝
- Filled out completely and signed.

3. 𝑷𝒂𝒔𝒔𝒑𝒐𝒓𝒕-𝒔𝒊𝒛𝒆𝒅 𝑷𝒉𝒐𝒕𝒐 🖼️
- One passport-sized photo (3.5 cm x 4.5 cm), taken within the last six months.
- The photo must have a white background and make sure na gandahan mo kasi yan na ang magiging picture mo sa visa mo.

4. 𝑩𝒊𝒓𝒕𝒉 𝑪𝒆𝒓𝒕𝒊𝒇𝒊𝒄𝒂𝒕𝒆 📜
- Must be issued by the Philippine Statistics Authority (PSA) within one year of the visa application.

5. 𝑴𝒂𝒓𝒓𝒊𝒂𝒈𝒆 𝑪𝒆𝒓𝒕𝒊𝒇𝒊𝒄𝒂𝒕𝒆 (𝒊𝒇 𝒂𝒑𝒑𝒍𝒊𝒄𝒂𝒃𝒍𝒆) 👰🏻‍♀️

🚨NOTE!! 🚨
⚠️If Birth/Marriage Certificate is unreadable, submit Birth/Marriage certificate issued by Local Civil Registrar.
⚠️If Birth Certificate is “LATE REGISTRATION”, submit Baptismal Certificate AND School Record (Form 137).
⚠️If there is no record of Birth/Marriage in PSA, submit Birth Certificate issued by Local Civil Registrar and Negative Certificate issued by PSA.

6. 𝑰𝒕𝒊𝒏𝒆𝒓𝒂𝒓𝒚 🧳
- A day-by-day travel plan (your intended activities, destinations, etc.)

7. 𝑩𝒂𝒏𝒌 𝑪𝒆𝒓𝒕𝒊𝒇𝒊𝒄𝒂𝒕𝒆 𝒘𝒊𝒕𝒉 𝑨𝑫𝑩 💰
- A recent certificate showing proof of sufficient funds to cover your stay.
- If the Average Daily Balance (ADB) in the last six months is not indicated, bank statement must be submitted to prove transactions within the last six months.

8. 𝑻𝒂𝒙 𝑷𝒂𝒚𝒎𝒆𝒏𝒕 𝑪𝒆𝒓𝒕𝒊𝒇𝒊𝒄𝒂𝒕𝒆 🏦
If EMPLOYEE: Income Tax Return (ITR) or Form 2316 (latest copy)
If Business Owner: proof of actual payment of tax must be submitted in addition to the BIR Tax
Payment Certificate Form

9. 𝑷𝒓𝒐𝒐𝒇 𝒐𝒇 𝑬𝒎𝒑𝒍𝒐𝒚𝒎𝒆𝒏𝒕 👨🏻‍💻
- For employees: Certificate of Employment (COE) stating your position, salary, and tenure.
- For business owners: DTI “Certificate of Business Name Registration” and Mayor’s Permit

𝑨𝒅𝒅𝒊𝒕𝒊𝒐𝒏𝒂𝒍 𝑹𝒆𝒒𝒖𝒊𝒓𝒆𝒎𝒆𝒏𝒕𝒔 (𝒊𝒇 𝒑𝒂𝒓𝒕𝒊𝒂𝒍/𝒇𝒖𝒍𝒍𝒚 𝒔𝒑𝒐𝒏𝒔𝒐𝒓𝒆𝒅):
1. “Letter of Guarantee” from the sponsor.
2. Sponsor's proof of relationship (Birth certificate, photos, etc)
3. Sponsor’s bank certificate, ITR, and COE.
