FOR PASSENGERS DEPARTING FROM THE PHILIPPINES:
If you are departing from the Philippines, you must accomplish the Passenger Profile and Health Declaration (PPHD) form online via https://bit.ly/PALIntPPHD as early as 5 days prior to your flight departure.
Exempted from accomplishing the online PPHD form are travelers bound for the United States, Canada and Australia, all of whom must accomplish the paper form upon check in at the airport. (Passengers bound for Singapore are required to accomplish the PPHD paper form upon airport check-in in addition to the online registration.)
Passengers departing on PAL international and domestic flights from Manila may undergo RT-PCR testing at the Philippine Airlines Learning Center (PLC) located at 540 Padre Faura cor. Adriatico St. Ermita, Manila.
The PAL center provides drive-thru or walk-in RT-PCR testing from 8AM to 5PM daily at a cost of PHP4,000. Results are released within 24 hours.
Upon entering the PAL Learning Center for your RT-PCR testing, please present your ID, plane ticket and QR code indicating that you have successfully registered the passenger profile and health declaration online.

FOR PASSENGERS ARRIVING IN THE PHILIPPINES:
Passengers arriving at Manila airport are required to register at least three (3) days prior to their flight departure for the mandated COVID-19 swab test (RT-PCR) to be administered upon arrival.

Fill out required fields and click submit.
Upon submission, a confirmation with instructions will be displayed.
Attach a copy of RT-PCR Negative Result with date of extraction within 72 hours to date of travel, a copy of confirmed booking slip/form, roundtrip flight/travel details and proof of identification with Philippine Residency and send to [email protected] using the email subject OHDC-FamilyName, FirstName. Ensure that you are getting your RT-PCR Tests in accredited laboratories here: (https://hfsrb.doh.gov.ph/?page_id=1729) and your hotel booking in DOT Accommodation Establishments with CAO/PCAO in the Facebook Account of DOT VI Western Visayas (https://www.facebook.com/DOTR6WesternVisayas/photos/a.1697814170399742/1697813293733163). Multiple tourist documents can be sent in one email. Just include a listing of the names of the tourists. Note: Please submit your documents at least 12 hours or more before your flight/travel to avoid delays. Please send only valid documents as falsification is punishable by law.
Wait for the email response (successful or unsuccessful) of the Final Validator. The validation team processes the QR Code within several minutes from receipt of email and usually responds within an hour. If you have not received any response within two hours, please send another follow up email.
If processing is successful, a copy of HDC with Tourist QR Code will be sent back. Keep the Tourist QR Code with you at all times. Have a printed or digital copy. The QR Code will be used in the ports of entry/exit (land, air, sea) as well as in accessing services in Boracay Island.
If processing is unsuccessful, an email will be sent to you requesting for additional requirements or clarification. Please immediately comply with this to avoid delays in the QR Code release.
Your Tourist QR Code has an expiry of twelve (12) hours from your accommodation checkout time. If you are extending your stay in Boracay Island, please send an email of a new booking slip showing the extension to [email protected] before your original checkout time.
