Boracay Travel Requirements Checklist (as of November 2021)

• For unvaccinated individuals: Negative RT-PCR Test Result
• For vaccinated individuals: Vaccination Certificate via (starting November 16, 2021)
• Roundtrip Flight Tickets
• Confirmed Accredited Hotel Booking
• Health Declaration Form for Boracay Tourist QR Code
• Government Issued-ID

UPDATE as of November 2021: No more negative RT-PCR test results required for fully-vaccinated individuals starting November 16, 2021.
Fill Out Health Declaration Form
Visit https://www.touristboracay.com/ and select BORACAY to access the official health declaration form. This will need to be completed as part of the necessary travel requirements.
Travelers will need to disclose personal information, valid ID information, details of Philippine residence, flight and hotel booking details, travel history, and itinerary.

And likewise, those individuals who have received their respective single-dose vaccines, such as but not limited to Johnson and Johnson, two (2) weeks before the day of travel.

COVID-19 Health and Travel Requirements
TOURIST (Point-to-Point/Bubble Tourism)
Who are allowed:
• All FULLY VACCINATED domestic tourists and foreigners (particularly those already staying in the
Philippines before the start of the pandemic).
• Minors accompanied by parents; legal guardians are allowed.
• Fully vaccinated individuals are those who are on their 15th day after receiving their respective 2nd dose of their vaccines. And likewise, those individuals who have received their respective single dose vaccines, such as but not limited to Johnson and Johnson, two (2) weeks before the day of travel.

ENTRY REQUIREMENTS FOR TOURISTS:
• Valid ID
• COVID-19 Vaccination Card.
• Copy of Negative swab-based QR Coded RT-PCR Test Result or Saliva Test conducted by Philippine Red Cross
with official receipt taken within 72 hours from time collection of specimens prior to boarding.
• Confirmed hotel, tour or diving booking/itinerary
• S-PASS
ROF (Returning Overseas Filipinos)
• Present vital documents issued by the National Government, including RT-PCR results.
• Fully vaccinated ROFs with negative RT-PCR test result shall no longer be subjected to the facility-based quarantine.
• All non-fully vaccinated and unvaccinated ROFs will be required to submit themselves to the 7-day mandatory
facility-based quarantine protocol. Said ROFs shall undergo an Antigen Test on day 7.
• All ROFs that will be subjected for quarantine inside a BQF or through a facility-based quarantine
(as pre-determined by EOC Coron) for 7 days shall be the one to shoulder all pertinent expenses.
CULION
LSI/ RETURNING RESIDENTS, ROF, and APOR
• Valid ID
• Approved S-PASS (TCP)
o Negative RT-PCR/ Saliva (Philippine Red Cross) Test Result conducted within seventy-two (72) hours with Official Receipt (for LSI/ Returning Residents and APOR)
o Certificate of Quarantine from BOQ (for ROF)
o Company Travel Order (for APOR)
• Stay Safe App/ QR Code
• Acceptance Letter from EOC Culion
• Vaccination Card (for fully vaccinated individual)

TOURIST
• Valid ID
• Approved
• S-PASS (TCP)
• Stay Safe App/ QR Code
• Acceptance Letter from EOC Culion
• Vaccination Card with QR Code (only fully vaccinated individuals are allowed to travel)
