1 photocopy of philhealth ID
1 photocopy of any valid ID
Reminders:
Please schedule 1-2 weeks ahead before your preferred date. Mabilis po maubos ang slot.
Call their numbers to secure a slot, at p**i handa po ang philhealth number dahil kukunin po nila ito kasama ng inyo basic information.
Siguraduhin may valid email address, dito na din po isesend and result ng inyong PCR test.
Kailangan pong may sasakyan (pwede ding single na motor) kung walang sasakyan, may narerent po tricycle or motor sa malapit, rates start at Php 100.
Strict po sila, kung anong date ka naka sched doon ka lang pwede mag pa PCR test. may list sila ng names para sa mga naka schedule for the specific date. Kung walang ka sa list, papauwiin ka lang din.
Bukas ito sa mga hindi taga-Maynila basta may appointment at 100 katao kada araw lang ang pwedeng magpa-test.
