"after investigators determined the airline was at fault" - not exactly true... they were fined because the airline could have provided more training. Employees were told they should not approach the plane.

OSHA fined Piedmont Airlines, an American Airlines subsidiary, after investigators determined the airline was at fault in the death of 34-year-old Courtney Edwards in Alabama.
