Experience Health ND Experience HealthND is a North Dakota Medicaid benefit for recipients with chronic health conditions.

Conditions covered by the program include asthma, chronic obstructive pulmonary disease, diabetes and heart failure. The program is voluntary, confidential, and free to eligible recipients. After you enroll in Experience HealthND you can call a nurse for information and assistance at any time of the day, everyday! A Registered Nurse calls or meets with you to learn what your needs are and prepares

a care plan just for you. Working with you and your health care provider, your nurse will provide information, education that you can use to manage your health condition, assistance in finding services and other support that helps you follow your doctor's treatment plan. You and your nurse work together to use these useful Experience HealthND services:

•A toll free number, 1-866-435-4306, you can call 24 hours a day, 7 days a week to speak with a nurse about your health concerns
•Help in finding a doctor or coordinating with your doctor and other health care providers to get the most from your care
•Education about choices you can make to improve your health
•Information sources and education about how medicines, exercise, nutrition, recreation, rest, and other factors affecting your health and how well you feel. To Enroll:

Held March 27, 2018, American Diabetes Association Alert Day is a day to sound the alarm about the prevalence of type 2 diabetes by asking everyone to take the Type 2 Diabetes Risk Test. Tip Twelve: "Eat healthy, stay active. Eat fruits and vegetables which pack nutrients and help lower the risk for certain diseases. Limit your portion sizes and foods high in fat, salt, and sugar.
