VAXCERT ONLINE ISSUANCE GUIDE‼️
1. Go to https://vaxcert.doh.gov.ph and click continue.
2. Enter details of your vaccination status. Select whether you are Partially Vaccinated or Fully Vaccinated, and if you are vaccinated more than 48 hours for any of your doses.
3. Complete your personal information.
4. Enter your vaccination details.
5. Enter the 7-character CAPTCHA and click OK.
For "Incorrect Record"
1. If record is incorrect, please click "update record" below.
2. Fill in the correct information details.
3. Upload a clear copy of vaccination card and any government ID.
4. Click "submit request".
5. Wait for 3-5 working days (vary to nature of complaint) to re-apply.
For "No Record Found"
1. Click continue.
2. Upload a clear copy of your vaccination card and any government ID.
3. Fill in your email and contact number.
4. Click "submit request".
5. Wait for 3-5 working days (vary to nature of complaint) to re-apply.
