Structural collapse of what appears to be a steel-framed glass roof under construction in WA. One fatality has been reported.

One person is dead and at least one more is injured after a building collapsed at Curtin University in Perth, with emergency services still trying to locate any other people in the rubble.

Protection Work Meditations #364:

Failure to appropriately design and implement sufficient protection work measures can potentially lead to disastrous consequences for all parties involved.

Excavation work with steep unsupported soil batters adjacent to boundaries presents an unnecessary ‘real’ risk of soil collapse within the excavations’ influence zone, particularly during inclement weather.

This risk can be significantly mitigated by the provision of a site retention system along the boundary to retain the adjoining ground from the intended excavation work.

Another enjoyable CROSS newsletter! I've been involved in several glass failures investigations and I agree that not all are related to Nickel Sulphide (NiS) inclusions in the glass. Generally, NiS related failure manifests in a spectacular 'butterfly' shaped crack propagation of the glass which initiates at the exact location of the NiS stone (refer comment with image of typical crack propagation).
The support conditions of glass are often poorly detailed with respect to permitting differential thermal expansion and contraction of the glass and steel fixings, and sometimes lack of consideration of long term effects from exposure to the exterior elements.

Structural failure of a freeway sign in Melbourne. Most likely the result of inadequate installation of the bolted fixed connection (NB: bolts and plates are visibly still in tact following collapse). Very lucky this didn't result in a fatality.
