৩। ব্যবসায়ীদের জন্য ট্রেড লাইসেন্স ইংরেজীতে অনুবাদ সহ নোটারী কপি, কোম্পানী প্যাড ও ভিজিটিং কার্ড ইংরেজিতে,।
৪। চাকুরীজীবীদের জন্য অফিস থেকে এনওসি লেটার, ভিজিটিং কার্ড ও অফিসের আইডি কার্ডের কপি, Salary Account Statement & Solvency অথবা পে স্লিপ।
৫। ছবি ১কপি 35x45mm সাইজ রঙ্গিন, সাদা ব্যাকগ্রাউন্ড, ম্যাট প্রিন্ট।
৬। ন্যাশনাল আইডি কার্ড-NID (যদি থাকে)।
৭। টি আই এন (TIN) সার্টিফিকেট (যদি থাকে)।
৮। এসেট ডকুমেন্টস (বাড়ি, ফ্লাট, গাড়ি, ফিক্সড ডিপোজিট-FDR ইত্যাদি, যদি থাকে)।
৯। স্পাউস থাকলে তার পাসপোর্ট / ন্যাশনাল আইডি কার্ডের কপি এবং ম্যারিজ সার্টিফিকেট।
১০। সন্তান থাকলে তাদের পাসপোর্ট/ জন্ম সনদ/ ন্যাশনাল আইডি কার্ডের কপি। অধ্যয়নরত হলে স্কুল/কলেজ/ভার্সিটি আইডি কার্ড কপি।

For Medical

In case of medical treatment, a copy of the appointment letter from a hospital in Thailand and if applicable, an original letter from a local doctor/hospital if the applicant is a first-time traveler.

Recent case summary of the patient’s medical reports issued by the local hospital;

Copy of recent certificate of acceptance for treatment/invitation letter from the receiving hospital in Thailand confirming the patient’s condition and the necessity of transfer to Thailand. The letter should mention the accompanying person name(s) & relation, who will travel with the patient by Air Ambulance;

Booking confirmation letter (such as Med letter & Action Plan) of an Air Ambulance.
