Brief History
Care Connect was established more than 20 years ago with the aim of providing high standards of personalised domiciliary care in Northwich and the surrounding area. We work alongside social services, as well as providing bespoke private care packages. We are a family-run business, which boasts a dedicated, caring and compassionate team of carers and an efficient and experienced manag

ement team. All members of staff are trained to the highest standards and we go above and beyond to ensure our clients receive the best possible service. We provide a broad spectrum of services and every care package we offer is tailored to meet the needs and preferences of the individual client. Care Connect is a family-run business with over 20 years experience in providing domiciliary care in and around the Northwich/Middlewich area. We also provide care for clients across Cheshire, such as Northwich, Middlewich,Holmes Chapel,, Goostrey, Knutsford and Congleton. We have a highly experienced and devoted team of carers and managers, who can provide all forms of home care from personal care and companionship, to administration of medication. Each care package we provide is specifically tailored to the needs of the individual client and we strive to ensure that every client receives a service, which not only satisfies their needs, but also enables them to enjoy greater independence at home. We work closely with service users, their families and health and social care professionals to create bespoke care plans, which go above and beyond basic health needs; we are always willing to go the extra mile to ensure our clients feel valued, respected confident. Care Connect was assessed as providing a good quality service to our clients at our most recent Care Quality Commission assessment.

Job Description

· Rotas – completed efficiently and in a timely manner while considering geographical locations, travel time, time sensitive medication and carer/client compatibility.

· Identifying gaps in the rota and ensuring bids are placed on potential packages via brokerage systems.

· Building new runs to ensure continuous growth within the business while maintaining care staff contracted hours.

· Organise shadowing for new staff and plan availability according to the requirements of the rota.

· Complete spot checks including medication competency check to ensure policies and procedures are followed while identifying level of competency or further training needs.

· Provide support and advice for clients, family and staff during office hours and when managing the on-call.

· Assessments of clients’ needs and identifying any associated risks.

· Managing the timely reviews of care assessments.

· Complete and maintain care plans which are detailed, and person centred.

· Liaising with health care professionals and reporting deterioration in client’s health in a timely manner.

· Follow up with health care professionals in a timely manner to resolve all concerns and requests for support.

· Attend MDT meetings to achieve best outcomes for the service user.

· Manage on-call to ensure support for service users/family/carers and contacting the relevant professionals for medical intervention when appropriate.

· Participating in care visits during staff shortages to ensure the needs of the rota are met.

· Submit safeguarding and care concerns in a timely manner.

· Record and document all relevant information within the software system.

· Resolve all ‘alerts’ in a timely manner.

· Any other duties as directed by line mangers.
