An Easy Guide
Private sector workers who suffer from
work-connected sickness or injury
resulting to disability, or their families in
case of work-connected death, may file
claims for Employees’ Compensation
benefits at the Social Security System
(SSS). These benefits are in addition to
the PhilHealth or the SSS benefits.

Here are the requirements for filing an Employment Compensation Claim:
1. Certificate of Employment indicating the position and job description during time of illness / accident / death.

2. Certified True Copy of the page of the security or company logbook pertaining to the illness / accident of the employee or equivalent.

3. Medical abstract or certification from the attending physician or hospital where the employee was admitted / hospitalized.

4. IF NAGKASAKIT - Pre-employment medical checkup of FIT TO WORK certification from the employer and other medical records.

IF NAAKSIDENTE – Copy of the accident report if it happened inside the company grounds or facility. Police report if it happened outside the company.

IF NAMATAY – Death certificate, medical records, accident report leading to death. If married with children, bring marriage contract, birth certificates of children below 21.

5. FILL UP APPROPRIATE FORMS from SSS (nagkasakit or naaksidente / nabaldado / namatay ).

6. SUBMIT

7. If approved, a check will be issued in your name, IF DENIED we will assist in the motion for reconsideration.
