Requirements:
VISA REQUIREMENTS:
* For Employees (Frequent travelers)
1. Application Form
2. 1pc. Passport size colored picture
3. Passport Original (6 Months valid)
4. Copy of Passport First Page
5. Original & Copy of valid visa s and arrival stamps to OECD member countries for the past 5 years
(USA and other visas like Canada, Japan, Australia, New Zealand and Korea).
6. Employment Certificate Original (must include applicant’s position, date hired, compensation, address, office landline & email address).
(Mobile number not allowed).
7. Bank Certificate Original (must include account type, current balance, account opening date, 6mos average balance).
8. Original ITR / BIR form 1701. (With personal Tin Number)
ADDITIONAL REQUIREMENTS EFFECTIVE: March 01, 2015.
9. Bank Statement (original or certified true copy of bank statements/passbook for the last 3 months).
10. Leave of absence certificate.
11. Marriage contract -NSO (photocopy)
12. Birth certificate -NSO (photocopy)
* For Businessman
1. Application Form

7. Business Permit or Mayor’s Permit (Copy)
8. Personal Bank Certificate Original (must include account type, current balance, account opening date, 6mos average balance).
9. Original ITR / BIR form 1701 (with personal Tin number).

• Make sure that the back page of the passport is completely filled-up (address, tel. numbers and name of person to be contacted in case of emergency.
