1. Application Form (A4 size only) - can be downloaded at smbtravelph.com/japanvisa
2. Original Passport with more than 6 months validity
3. 2 pcs Colored Photo (4.5cm x 4.5cm) with white background
a. must be taken within 6 months
b. Please write applicant’s name and birthdate on back side of the photo.
c. Photo must be pasted on the application form
4. Birth Certificate must be issued within one year from PSA Main Office
5. Marriage Certificate must be issued within one year from PSA Main Office
6. Day to Day Itinerary in Japan
7. Original Bank Certificate - must include account type, current balance, account opening date and average daily balance
8. Bank Statement for the past 3 months - original or certified true copy of bank statements/passbook
9. Photocopy of ITR (Income Tax Return) or Form 2316
10. If with Guarantor who lives in the philippines, he/she will provide 7-9 requirements
a. Guarantee Letter
b. Proof of relationship between applicant and guarantor (e.g. Birth Certificates etc.)
Additional Requirement For Employees:
1. Original Certificate of Employment - must include the following
a. applicant's position
b. date hired
c. compensation
d. office address
Additional Requirement for Business Owners:
1. Photocopy of Business Registration from SEC or DTI
2. Photocopy of Business Permit or Mayor's Permit
