âïž Accomplished Application Form (will provide this) - must be originally signed by the applicant
âïž Daily Schedule in Japan (will provide the format) - must show the day-to-day activity including the date of arrival and departure and hotel address and contact details.
âïž Original Philippine Passport's -page 3 with signature (with at least 6 months validity prior your date of travel)
âïž Colored Copy of passport size (45mm x 35mm) Photo (MUST be taken within 6 months)
âïž Original PSA Birth Certificate and Marriage Certificate-if married must be issued not more than 1 year by PSA.
âïž Bank Certificate (valid within 3 months from the date of issuance of the bank), should be strictly attention to Japan Embassy and must be a withdrawable personal account.
âïž Any Financial Ties (Bank Certificate/Bank Statement, Payslip, COE, Approved leave of absence from employer, Company ID, Insurance and/or Investment - if any, Business Registration - if any and the like)
âïž Any Economic Ties (home ownership-land/house title, Lease of Contract-if lessor, Car and/or motor vehicle registration (OR/CR), and the like).
âïž Social and Community Ties (immediate family members not travelling with you, marriage certificate-if married, educational ties-if student, Contract of Employment-if employed and the like)
âïžPrevious travel stamp and/or VISA out of the country.
