Bluestone Physician Services is the medical provider for a large percentage of our residents at ACR. Bluestone’s model is tailored to patients living with multiple chronic conditions and disabilities. Bluestone recognizes that patients need a customized approach to care that is preventative, proactive and includes all members of the care team.

What makes Bluestone care even more incredible for our residents is that we currently have FIVE former ACR employees who are now providers at Bluestone including a former ACR RN, former Top 10 Staff of the Year, and a former Residential Supervisor! 4 serve as Physician Assistants and one as a Nurse Practitioner! We are so proud of you all and grateful to have been a part of your professional journeys!

One of the main functions of the Direct Care Professional job is to help residents through their daily routines/activities of daily living!

That involves:
✅ Helping residents get up and get ready for the day

✅ Preparing meals and assisting residents during mealtimes

✅ Enjoying fun activities together such as playing games, reading, doing crafts, baking, and more

✅ Advocating for residents out in the community

✅ Assisting with physical, occupational, and/or speech therapies

It’s truly a pleasure being able to respectfully and thoughtfully walk the residents at ACR Homes through their lives!

Activities of daily living, or ADLs describe the everyday self-care tasks that people complete such as eating, getting dressed, using the bathroom, or other things related to personal care.
