Basic Documentary Requirements:
1. Certificate of Eligibility from LSWDO or Certificate of Indigency from BLGU
2. Endorsement from the SUC
3. Valid Identification Card issued by any Government Office
4. Any proof of completion or graduation in the identified SUC (i.e, diploma, certificate of graduation, Transcript of record, yearbook, endorsement from SUC etc.)
5. Accomplished Health-Declaration form and CFWP Profile Form
6. Accomplished Disaster Profiling Tool
