Americare Home Health Services is an array of holistic services that encompasses health promotion and teaching, curative intervention, rehabilitation support and maintenance, social adaptation and integration and support fo
r the patient and family and appointed caregivers. Our physicians can take care of your healthcare needs in the comfort of your very own residence and will recommend a patient centered care plan based on their clinical findings and diagnosis. A full support team of nurses and therapists, including physiotherapists, occupational therapists, speech language pathologists, social workers and nutritionists/ dietetics will be available so all of your needs can be met. If requested, our physicians can become your primary care physicians or work with your doctor as a secondary physician during times when you may not feel well enough to visit your doctor in his/her office. Information will be exchanged with your primary care physician or your specialist upon request. Americare Home Health Services will accommodate all referrals by arranging for one of our physicians and a nurse to meet with the patient and family to discuss the health services to be implemented in the home.
