I want to share the process of applying for my daughter’s passport book and passport card, since it’s a little different than applying for my own. The total time to apply I want to was about 3 hours altogether on a Saturday. This timeframe will depend on how busy the select places are.

1. I filled out the passport application online, utilizing the form filler, on my daughters behalf (you can also pick up a copy at your nearest passport agency) https://pptform.state.gov/

2. I filled out the Statement of Consent Form and gave a photo ID of my license ( I wasn’t present during the submission of the application) https://travel.state.gov/content/travel/en/passports/need-passport/under-16.html

3. Took the Statement of Consent Form to UPS to be notarized $10 (they should have a notary on hand everyday) https://www.theupsstore.com/tools/find-a-store

4. Went to CVS to get the passport photo taken for $16.99 (passport photos can be done at a passport agency as well)

5. Took all the completed forms and my daughters Birth Certificate to a passport agency, in our case it was the post office. (You will have to locate a agency near you, because all USPS do not handle passports) https://iafdb.travel.state.gov/

6. Pay the required fees at the agency. In our case, a Passport book + Card + Ex*****on Fees + expedited fees came out to be $212

*These are the steps I took for my OWN child, everyone’s steps will not be like this, take your time to read and figure out what you need to do for yourself or family member*
