Monochromatic Infrared Photo Energy versus Low Level Laser Therapy in Patients with Knee Osteoarthritis.

Background


Objective
To compare the effects of MIPE (monochromatic infrared photo energy) and LLLT in improving pain and function in KO.

MIPE: Anodyne Therapy System: a flexible pad containing 60 LEDs

Study design & methods
Randomised comparative study.

Subjects: 60 patients with unilateral knee OA.

Methods: subjects were randomly assigned into two groups.
•Group 1 (n=30) received MIPE and exercises. MIPE consists of LEDs located on flexible pads, providing a total of 62.4 Joules/cm² of energy density during 30 minutes irradiation time.
•Group 2 (n=30) received LLLT and exercises. A Chattanooga 100mW, 850nm diode was used. 5 points were irradiated for 2 min per point = 12J/point. Total treatment time per session: 10 minutes.

Both groups received 2 treatments per week for 6 weeks.

Outcomes:
•Pain intensity measured on a 10cm visual analogue scale (VAS)
•Physical function measured with the lower extremity functional scale (LEFS)

Outcomes were assessed before and after the 12 therapy sessions.

Results •All outcome measures improved significantly in both groups, with no significant difference bewteen the groups.

Effects of therapeutic ultrasound on range of motion and stretch pain.


Objective :
To clarify the effects of therapeutic ultrasound on range of motion and stretch pain and the relationships between the effects.

Study design & methods :
Randomised controlled trial with repeated measures design.

Subjects: 15 healthy males.

Methods: subjects performed all three interventions:
•Ultrasound (US group), US was applied at 3 MHz with an intensity of 1.0 W/cm² and a 100% duty cycle for 10 minutes.
•Without powered ultrasound (placebo group),
•Rest (control group).

Outcomes:
•Active and passive range of motion (ROM)
•Stretch pain (VAS)
•Skin surface temperature (SST).

Results •ROM and SST were significantly higher in the US group than in the placebo and control groups for the 20 minutes after ultrasound.
•There was no change in stretch pain indicating that ultrasound increased the threshold of stretch pain.
•The SST increased with ultrasound and decreased afterwards. Additionally, the SST tended to return to baseline levels within 20 minutes after ultrasound exposure.

Conclusion •Therapeutic ultrasound provided increases in both ROM and SST accompanied by an increased SP threshold for at least 20 minutes after the conclusion of the interventions.
•Increases in tissue viscoelasticity and extensibility caused by an ultrasound application, which improves scar tissue and fibrosis, leads to greater ROM and less pain, and thus it is clinically important.
•Furthermore, our results endorse the active application of pain management and therapeutic exercise such as ROM exercises by therapists in the 20 minute window after application of ultrasound.

Key message Ultrasound has a thermal effect on the tissue and allows for greater passive ROM gains without pain increase.
